AFFIDAVIT OF _________________

I, JULIET V. PADERNAL, of legal age, Filipino and a resident of


Magatas, Sibulan, Negros Oriental, after having been sworn in
accordance with law hereby depose and say:

1. I am a faculty member of the Department of English and


Literature. I handle both undergraduate and graduate
courses. I have served as Chairperson and Language
Programs Coordinator of the department.

2. In the second semester of 2005-2006, I was requested to


review Ms. Sheila T. Riveras papers that she had submitted
to the late Dr. Imelda Y. Flores. Ms. Rivera submitted her
papers when Dr. Flores was still alive.

a) The grading sheet indicates that Dr. Flores handled EL


30 in the second of SY 2004-2005.

b) Ms. Sheila T. Rivera was enrolled in Dr. Flores EL 30


class at that time. Her grade was INC.

c) Ms. Rivera is a graduate student [MA English (ELS)] who


was advised to take EL 30 as an undergrad requirement
because her Bachelors degree was not in English.

d) Ms. Rivera had submitted her EL 30 papers to remove


her INC mark within the specified period, but
unfortunately, Dr. Flores died before the completion
form was submitted.

3. After reviewing Ms. Riveras completion papers and


consulting with the other program faculty members, I gave
Ms. Rivera a grade of 3.7, filled out the necessary forms, and
submitted these to the office of the English & Literature
department.

4. Shortly after, Ms. Rivera took some parts of her


comprehensive exam, which can be taken only after all the
grades in the academic coursework are complete.
5. I am executing this affidavit to attest to the truth of the
foregoing facts.
