Pt. is a 4 y/o female with Spina Bifida. Pt. engaged in a 1 hr. cheerleading session on

10/25/2011.

Subjective Data: Pt. showed awareness of needs, AEB asking guardian to dry their hair.

Pt. also showed awareness of capabilities, AEB refusing help c shoes, stating, Ill do it!

Objective Data: Pt. used w/c to amb & required Total Assistance to Mod I Assistance c

ADLs. Pt. demonstrated little to no control over BLE but had FROM of BUE. Pt. appeared

to be A&Ox3.

Analysis: Pt. did not appear deterred by Spina Bifida-related difficulties. Pt. was able to

communicate needs & have + social life through cheerleading. Pt. able to participate in

activities c Total Assistance to Mod I Assistance from guardians & w/c.

Plan: Pt. will participate in a cheer routine designed to engage UE & ability to move

around on floor.

Intervention Implemented: Pt. engaged in a 1 hr. TR session, participating in a cheer

routine that engaged pt.s strengths & weaknesses.

Evaluation of Response: Pt. responded + to intervention, able to participate more

comfortably c peers but UE grew fatigued as session progressed.

Revisions: Pt. will participate in more strength-building activities to make cheers less

tiring.
