AFFIDAVIT OF SERVICE

I, AMADIO T. PANAY, of legal age, Filipino, married, and a resident of

Washington St., Talomo, Davao City, after having been sworn to in accordance with
law, do hereby depose and state that:

1. I am the same person who caused the execution of this affidavit;

2. I am a member of First Word Christian Fellowship (FWCF) under the

leadership of Rev. Remecio Malicong;

3. I was requested by Mrs. Aida Gocela to serve a demand letter to Rev.

Malicong which I did on November 29, 2015;

4. I gave a copy of the Demand Letter to Rev. Malicong but he refused to affix
his signature on the Received by portion of the letter;

5. Rev. Malicong handed a copy of the said Demand Letter to Ms. Zenith
Bagwasan, one of the Church members, for the latter to be read to the whole
congregation of (FWCF);

6. The said Demand Letter was read to the congregation and in my presence
on November 29, 2015, but after the reading, Rev. Malicong returned the letter to me
and refused to retain a copy of the same;

7. The letter that was read was a demand for FWCF to vacate the property of
Mrs. Gocela, on which the church building is located;

8. I am executing this affidavit to attest to the truth of the foregoing fact of

service for whatever legal purpose this may serve.

IN WITNESS HEREOF, I have hereunto set my hands this 11 th day of

December 2016 at Davao City, Philippines.
