CONFIDENTIALITY

PLEASE BE INFORMED!

This is a note to all of my clients to remind you that all information shared in
individual or joint sessions with me remains confidential except in the
following circumstances:

1. If you are about to seriously harm yourself or someone else

2. If you inform me of a child being abused (physically, sexually or

otherwise) or a child in need of protection-only a suspicion is
necessary!

3. If you inform me of sexual abuse or sexually inappropriate behaviour

by another professional (if you give me the name of the person) (I dont
need to reveal your identity when this is reported)

4. If a court should subpoena my records

I am professionally and/or legally required to take action in the above

situations. Should any of these circumstances arise, I will discuss it with
you first.

Please be aware of the above and feel free to ask me any questions you may
have.
