Transition Outcomes

Employment:

ML is a high functioning student in many areas of transition. He has many interests


with a focus on trains. This is both in employment and recreation/ leisure. Through
personal interviews with MLand his mother it has been determined that he wishes to
obtain a career in the railroad industry. He hopes to be a train conductor eventually,
but would be happy to just be working around trains.

Objective:

After completion of the Bridges program and no more than ten years after, ML will
obtain a job within the railroad system that will challenge his independence and
vocational skills.

Supporting IEP goal:

ML will be able to fill out a job application by filling out all personal
information (full name, social security number, phone number, address, birthdate)
on 2 out of the 3 attempts.

Recreation/ Leisure

As mentioned above, ML has a passion for trains. This is shown through his current
hobby of model trains. He and his dad partake in a model train community and
travel together to go to model train conventions. When ML earns a pay check from
his current job (HEB), he likes to spend it on new model trains. Through interviews
of both ML and his mother, it was determined that ML would like to continue this
hobby when out of the Bridges program.

Objective:

After completion of the Bridges program within 6 months, ML will locate, join, and
actively participate in a local interest group/ hobby group that pertain to model
trains.

Supporting IEP objective:

ML will be able to use an internet search engine to locate an interest group,


within the Bryan/ College Station area, which pertain to model trains independently
5/5 times.

Independent living

ML one day plans to be independent, but he does not feel that this is in his near
future. He is aware of the many skills he still needs in order to live on his own. He is
realistic in his thinking of independent living after the Bridges program. ML is very
well educated in his surroundings but still struggles with receptive and expressive
language. For these reasons, ML also struggles with decision making. With all of
these factors in mind, ML wishes to live at home as he continues to gain more
independent living skills.

Objective:

After completion of the Bridges program, ML will live at home and participate to the
maximum extent possible in his daily routines (e.g. hygiene, housekeeping,
transportation, communication scheduling, laundry, cooking/ grocery shopping).

Supporting IEP objective:

ML will be able to create a grocery list of at least five items and locate them
in a grocery store (under 30 mins) independently, finding at least 80% of his items.

Education

ML is determined to receive education after the Bridges program. He initially


expressed his desire to attend TSTC in Waco. After further interviews with ML, he
decided that he would like to go to Blinn College for a year while living at home in
order to ease his way into higher education. He eventually would like to receive a
technical certificate/ degree to aid in his dream of being a train conductor.

Objective:

After completion of the Bridges program and by the following Fall semester, ML will
be enrolled in at least 3 hours at Blinn College in College station.

Supporting IEP Objective:

ML will be able to get to the Apply Texas website and sign into his account
independently 100% of the time.
