PROGRAM DESCRIPTION (INCLUDING TARGET POPULATION, LOCATION, DATES/TIMES)


The Dealing with Diabetes event is aimed at children and families of children aged 5 to 15 at risk for
diabetes and with diabetes. The event will be held at Mack Cook Stadium in Lenoir, NC on Saturday May
13, 2017 from 10 a.m. until 2 p.m. Participants will be able to visit a variety of educational areas and learn
more about healthy lifestyle choices that will help them live their best life. Lifestyle choices covered will
be eating habits, exercise, weight management, and monitoring glucose levels. Volunteers from local
health clinics, the diabetes center, and Quest For Life will provide information about healthy meal and
snack options, demonstrate and engage children in fun exercises to beat inactivity, and provide glucose
screenings. A physical activity area will be available for children to try out new exercises such as yoga or
to engage in playtime with other children. Glucose monitors will be given to participants through
donations from different glucose monitor manufacturers. Children attending the event will be included in
a drawing for different door prizes such as a bike and passes to Bos Entertainment. Each child will
receive a picnic kit for participating. Parent and children surveys will be conducted at the end and two
weeks after the event. This event is a joint project of UNC Healthcare, the Caldwell County Health
Department, and Caldwell County Schools.

GOALS & OBJECTIVES (SMART); AT LEAST ONE PROCESS & ONE IMPACT OBJECTIVE
Goal: To increase knowledge of healthy living lifestyles that have a positive impact on the health of children with
diabetes.

Objectives:
By November 15, 2016, county school nurses will be contacted to help identify and invite children at risk for
diabetes and with diabetes.
By the end of the event, 85% of children will be able to list two healthy snacks and reasons physical activity is
important.
Two weeks after the event, 50% of children surveyed will answer yes when asked if they have changed one or more
of the following habits: tracking glucose readings, swapping an unhealthy snack for a healthy one, or increasing
physical activity.

LIST INTERVENTIONS AND HEALTH BEHAVIOR CHANGE THEORY

1) Activities will provide children with the knowledge they need to believe they can take control of their health
(Health Belief Model).

2) The play area will highlight physical activity and children engaged in physical activity (Social Cognitive
Theory).

3) Each child will receive a picnic kit with backpack, flyer, and bike bottle for participating in the event (Behavior
Management).

4) Parents will learn along with their children and be provided with educational materials that will enable them to
provide support and assistance to their child after the event (Stage Theory).

EVALUATION PLAN (INCLUDES DATES; USE PROFESSIONAL TERMS)


Staff will conduct a summative evaluation on June 7, 2017. Surveys of children conducted at the end of the event
along with children and parent surveys conducted two weeks after the event will be utilized in the summative
evaluation. The end of event surveys will evaluate knowledge children gained regarding healthy eating, physical
activity, and the importance of glucose control. The follow up survey will demonstrate if children are practicing
what they have learned. Parent surveys will focus on satisfaction with the event and benefits to their child.

Staff will review the child and parent survey responses as well as conduct surveys of exhibitors, volunteers, and
partner organizations. Staff will meet to discuss the effectiveness of the event based on lifestyle changes being
made by the participants and satisfaction of parents. An evaluation summary will be shared with sponsors,
volunteers, community partners, exhibitors, and health care agencies.
