JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

RE- ONE AND THE SAME PERSON

WE EUSTAQUIA DADO CALESA and MELANIO GERVACIO TALLE, both of legal

ages, Filipinos, widow and married, respectively and both are residents of Sitio So-oy,
Inangatan, Tabango, Leyte and Brgy. Layog, Tabango, Leyte after having been sworn in
accordance with law hereby depose and state that:

1. We personally know MA. EMELOU R. PELAYO, who is our long time

townmate and acquaintance of Brgy. Campokpok, Tabango, Leyte, and the
fact of her birth is registered at the Local Civil Registrar of Tabango, Leyte.

2. We know for a fact that the person by the name of Ma. Emelou R. Pelayo as
appearing in some documents or specifically in her Certificate of Baptism,
Marriage Contract and identification cards and the name Emelou R. Pelayo
as appearing in her Certificate of Live Birth, refer to one and the same person
and that her true and correct name is MA. EMELOU R. PELAYO.

3. We are not in any way related to the foregoing person within the fourth civil
degree either by consanguinity or affinity, nor do we have any pecuniary
interest in this matter whatsoever.

4. To remove confusion as to the two names, we hereby declare that the two
Ma.Emelou R. Pelayo and Emelou R. Pelayo refer to one and the same person
whose registered name is Emelou R. Pelayo.
