Due to these facts the need of community-based services is extremely important,

that is one of the reasons la Edad de Oro is a key component of the Behavioral
Health program.
EDUCATIONAL TOPICS
Every month, we present a variety of educational topics of interest, such as:
o Healthy Choices
o Fall Prevention disappear
o Alzheimer's
o Emergency Properness
Some of these topics link with a monthly health observances and
celebrations
(National observances; Dental Hygiene Month, Domestic Violence Awareness, Talk
about Your Medicines, Halloween and Dia de los Muertos etc)
In addition, every third Friday of the month, Fernando Marquez MFT present
topics related to Behavioral Health and every forth Friday, Andres Bravo PA is
available to answer questions for senior participants on uncomplicated medical
issues.
PHYSICAL ACTIVITIES

o Tai-Chi Led by Jeannie Miller, FNP
o Stationary walking
o Zumba for seniors
o Chair Yoga
o Chair exercises for seniors
o Go4 life from the National Institute on Aging DVD
o Feeling Fit Club from the Aging & Independence Services DVD

PEER SUPPORT

Research has shown that peer support facilitates recovery and reduces health care cost,
increase social support and participation in the community, improve well-being, self-
esteem, and social functioning. Seniors provide assistance, through empathetic listening
and encouragement. This program is intended for seniors experiencing difficulty coping
with life stressors (e.g., illness, mild depression, isolation, loss, etc.).

CASE MANAGEMENT
The coordinator meets with seniors one on one to develop an individualized Care Plan
(ICP) and to identify areas of concern.

Seniors receive care coordination in relation to a variety of jointly identified issues such
as medi-cal, Medicare, housing, insurance, mental health, medical appointments and
other non-related health problems.

SENIOR PEER PROMOTORAS

The Promotora model;

Promotoras are trained in outreach and topics such as: health topics, HIPPA, (health
insurance portability and accountability act ). They also attend periodically to
educational workshops and annual conferences tailor to community workers.

The goal is to empower seniors citizen to live independent by educating the Spanish
speaking community.
