PETITION
PETITIONER, thru counsel and to the Honorable Court, alleges:
1. On _______, deceased ______ died on ______ in the City of Manila, then
residing at _______, Manila, which was his residence at the time of his
death, leaving a will, true copy of which is attached hereto as Annex A
and made an integral part hereof.
2. Petitioner, of legal age, and a resident at ________ Manila, and is the person
named as executor in said will, and he hereby consents to act as such
executor.
3. The names, ages, and residences of the heirs, legatees and devises of
said decedent are as follows:

5. Petitioner, as named executor, moves and prays that letters testamentary,

WHEREFORE, the petitioner prays that after due notice, publication,

and hearing the aforementioned will of deceased ________ be approved and
allowed, and that letters of testamentary be issued to herein petitioner.
