Authenticate your LONG FORM COLB with Secretary of State where you were born. Th
eir website will have cover page, fees, address and instructions. Remember you w
ant an Authenticated Certificate - not an Apostille. Certificate is for Non-Hagu
e countries, so on the form select a non-Hague country like Jamaica.
When you receive your Authenticated COLB Certificate back from your birth state
SoS, send the whole thing (do not take it apart) to the US Dept of State, Secret
ary of State with an online completed form DS-4194 and USPS Money Order for $8.0

This is where you will find the online DS-4194 that has the drop down boxes (see
photos). Also the USPS Money Order expedites their processing turn-around for y
ou.
The online editable form only works on a PC (NO APPLE PRODUCTS) with Internet Ex
plorer. Forget trying to access this with any kind of Apple product or another b
rowser like Crome or Firefox.
You want the Online Editable version. It is the only one that prints a unique ba
r code on it and has an expeditious return. If you choose to use another DS-4194
version of the form, you will wait for manual processing... automated processin
g with unique barcode 10-days; manual processing-up to six weeks.
You'll need to enclose a USPS Money Order for $8.00 per document you want Authen
ticated. I had a name change court order Authenticated. 1st with state where doc
ument was created & 2nd US Dept of State, Rex Tillerson. There are a multitude o
f documents that could be more helpful to once Authenticated via this Counter-De
ed Process... POA, Divorce Decree... See pictures of drop down boxes with list o
f the documents they Authenticate.
The COLB is a negotiable instrument, I it sent via USPS Priority mail & enclosed
a SAS-Priority Mail envelope for them to return it to me.
