I hereby accept the prize as awarded and such acceptance shall operate to release and forever discharge
Sony Music Entertainment (Canada) Inc., its predecessors, successors, affiliates, parent company and
assigns, CFOX RADIO, and A&B Sound, their advertising and promotional agencies and their
employees, representatives and agents, and their respective heirs, executors, administrators, personal
representatives, successors and assigns from any and all claims, demands, damages, actions, causes of
action, costs and payments whatsoever for damages, loss, injury or death howsoever caused from arising
from my accepting the Prize, or otherwise (including but not limited to any claims arising from the nature
of the prize and my participation in taking it, acts of God or unforseen circumstances) which I, my heirs,
executors, legal, personal representatives, administrators or assigns have, may have had or shall in the
future have against them or any of them in respect to the Contest, the conduct thereof, my participation
therein, the award of and use and enjoyment of the Prize, or otherwise.
