Unsafe lifting observed in lay down area, rigging team was not there, safety coverage,
lifting plan didnt seen. Workers were holding huge weight without tag lines and under
boom and suspended load.

1.un skilled crews were doing the critical job.
2.there was no safety coverage by contractor.
3.concern job training was not given.
4.This subject was not taken before in tool box meeting which should be conducted by
contractor HSE.
5.Lack of coordination.

Job had been stopped and explain to contractor assure the next time for this type of
activity should be done by certification holder crews. Before start the job HSE should
analyse the hazard and must be taken controlling measures
