AFFIDAVIT OF NON-FILING OF INCOME TAX RETURN

I, MA. CELESTIA CASIL-GONTIAS, in legal age, Filipino citizen, and

resident of Blk. 04 Dalanghita St. Panam Village Brgy. Pinagsama, Taguig City, after
having been duly sworn to in accordance with law, hereby depose and state:

1. That I am the parent of Marcelo C. Gontias.

2. That Marcelo C. Gontias is currently enrolled as Second year college

(Ladderized System) student at Technological University of the Philippines-
Taguig, Taguig City.

3. That my said son is applying in a Free Tuition Fee for all State Colleges
Program under the Commission on Higher Education (CHED).

4. That one of the requirements is a proof of the Bureau of Internal Revenue

(BIR) validated Income Tax Return (ITR) of parent/guardian.

5. That I did not file my ITR at the BIR because my income does not exceed Php
60,000.00, having only a small carinderia.

6. That I am executing this affidavit to attest to the truth of all the above facts
and statements for all legal intents and purposes it may serve.
