AFFIDAVIT OF DISCREPANCY

I, MARIO LOUMELE ESTRAERO, of legal age, Filipino, single and a

resident of #8 M, Ceniza St., Mandaue City, Cebu, after having been sworn in
accordance with law do hereby deposes and state THAT:

1. I am MARIO LOUMELE ESTRAERO born on May 27, 1985 at Ormoc,

Leyte as recorded in my certificate of live birth;

2. On my passport previously issued by the Department of Foreign Affairs

in Cebu City, my name and place of birth was mistakenly written and it
appears that my name was MARIO LOUMELE ESTRANERO born at
Mandaue City, Cebu, instead of MARIO LOUMELE ESTRAERO born
at Ormoc, Leyte as recorded in my certificate of live birth;

3. The names MARIO LOUMELE ESTRANERO born at Mandaue City,

Cebu and MARIO LOUMELE ESTRAERO born at Ormoc, Leyte
refers to one and the same person;

4. I execute this Affidavit of Discrepancy to attest to the truth of the

foregoing facts and to explain the discrepancy in my birth
certificate with regards my surname.
