Miss Sumner Contest Application

Applications are due by 4 pm on June 12th. Applicants must have a Sumner address and be actively
enrolled in school. Please provide a 4x6 or smaller photo. Contestants must be at least 16 years of age
and not more than 19 years of age on the first day of Iowa State Fair in this given year (August 10th).

It is understood that the winner of the contest is expected to participate in activities during Sumner Days (June 24) and the
Bremer County Fair. We, at no time, hold CEO Sumner, CEO Sumner judges, or any and all other persons associated with the
above listed contest responsible or liable for any personal injury or damages to property as direct or indirect response of
participation in said contest.

The information on the Miss Sumner Application will be used by the judges as part of their decision-making process to
determine who will become Miss Sumner. It is assumed by the judges that all parts of this application are true. Any
falsification of this form will be grounds for disqualification from this or any future Miss Sumner contests.
I hereby declare that any statements or other information provided in the Miss Sumner application represents the
absolute truth. I understand that any piece of false information disclosed on this application will disqualify me from this and all
future Miss Sumner contest.
In addition, if chosen to be Miss Sumner, I agree to make appearances at the 2017 Sumner Celebration. In the event
that I am crowned Miss Sumner, I will be available to attend the 2017 Bremer County Fair and run for the Bremer County Fair
Queen.
