The members wife called the health plan and told her husbands case manager how thankful she
and her husband were for Lynnes visit.
The visit wasnt an easy one. Lynne found the woman to be quite overwhelmed in her role as her
husbands sole caregiver. His situation was complex with COPD, uncontrolled diabetes, an enlarged
heart and chronic kidney disease being among his top concerns.
In addition to those existing issues, Lynne found swelling in both of his legs, a skin infection and
wheezes throughout all lung fields.
She promptly called the members primary care provider and subsequently sent the member to the
hospital where X-rays revealed he was in deep congestive heart failure (CHF). He was directly
admitted, treated and discharged.
The health plans case manager called Matrix to relay the good news, express the wife and
husbands appreciation and praise Lynne for her keen assessment skills.
