INFORMATION AND VERIFICATION APPLICATION FORM


Cost of verification = R250 per application. Cost of verification and motivation = R500 per application.

Complete SAPS 271 application form with a black pen.

Attach the following documents to the application:

1. Two (2) full colour passport photographs against a white background


2. Proof of payment
3. Signed indemnity form

Certified copies of the following supporting information, where applicable:

a) ID document.
b) SAHGCA member certificate.
c) Competency certificate and SASSETA or PFTC certificate.
d) SAPS Competency certificate.
e) Proof of residential address (e.g. copy of municipal account). If residing with someone else, attach a
sworn statement by the person and a proof of that address.
f) SAHGCA endorsement for the relevant firearm (See application form attached.)
g) Dedicated Hunter and/or Dedicated Sportshooter Certificates with updated Section 16 letters (sworn
statement) issued with certificates by SAHGCA.
h) Summary/copies of current firearm licences.
i) Detailed background of hunting and/or sportshooting activities: (Complete attached questionnaire in full)
j) Proof of participation in hunting and sportshooting activities.
k) Most recent certificates and awards achieved.
l) Letters, photographs, and permits of most recent hunting excursions.

1) If the firearm is taken over from a private person, provide the following documents:
a) Copy of current owner's firearm licence.
b) Copy of current owner's ID document.
c) Sworn statement in case of a lost firearm licence.

2) When purchasing a firearm from a dealer:


a) Completed SAPS 350(a) form and confirm that it is signed and dated.

3) When taking over a firearms from an estate:


a) The executor must complete the relevant sections of SAPD 271 and sign the document.
b) Letter from executor confirming his/her appointment as executor
c) ID of executor.
d) Letter for transfer/permission from executor letter
e) Death certificate of former owner.
f) Copy of former owner's ID document.
g) Copy of the original licence, if available.
Firearm Safe

Three (3) full colour photographs of safe.


1. One photograph of safe with door closed.
2. One photograph of safe with door open.
3. One photograph of safe showing bolts used to fix safe in position.

These must be FULL COLOUR PHOTOGRAPHS printed on ordinary white paper.


The initials, surname, ID number and signature of the applicant must be written on the back of the
photograph.
