    Sub : Change of Address prior to passport appointment

    Sir,

    with due respect I would like to inform you that I Dipak Maity had my residential address
    changed very recently from Flat but the address as provided by me and my wife Smt.Sima Maity
    while applying for an ordinary fresh passport was the previous and we uploaded the supporting
    address proof documents of the same.

    Our passport appointment is due on 12/03/2013 but our present address is as mentioned above i.e
    The photocopy of the Licence for occupation is attached along with this letter for your ready
    reference.

    Therefore I would request you to acknowledge this change in address and take necessary action
    while verifying the same.
