Dear Friends of the Dorset Community Health Care Hub,

Please consider the Hub in your planned giving this year. Your donations will help us
diversify and expand our ability to meet the needs in our community. While we
continue to offer patients better access to primary and preventative care with
expanded summer hours we have launched several initiatives to meet all
determinants of health. We are proud of the services offered including: a diabetes
prevention program, mental health therapy, diabetes days, crisis food program,
family/Christmas support programs, fall immunization days, healthy living programs
and a full slate of social supports and program offerings by our new outreach worker
now located in the Hub. With any donation over $20 we can provide a tax receipt.
