AFFIDAVIT OF NON-OPERATION

I, RICARTE G. GANOTISI JR., of legal age, Filipino citizen, and presently residing at
1328 San Diego St. Sampaloc Manila, after having been duly sworn to in accordance with
law, do hereby depose and say:

1. That I am the Owner/Manager of BIOGLASVSKI GENERAL MERCHANDISE, a single

proprietorship duly registered under the laws of the Philippines with principal office at #220
Phase 3 Barangay Burol 1Dasmarinas Cavite City.

2. That said Business was a Single Proprietorship on February 18, 2015 with COR Registration
date March 31, 2015 OCN: 1RC0000779903

3. That to date, the company has not commenced operations due to lack of business
accreditation as BIR-ICC as importer we need to accomplish first the BIR-ICC before we
proceed to AMO-BOC to be duly act as importer. Intend to commence operations in the
future.

5. That I am executing this affidavit in order to attest to the truth of the foregoing and for the
purpose of complying with the reportorial requirements of the Bureau of Internal Revenue.
