AFFIDAVIT OF DISCREPANCY

I, RUBY PANONG LAGARTO, of legal age, Filipino and a resident of

Dablo St,, Santiago (Pob), Philippines having been duly sworn in accordance
with law, hereby depose and state:

1. That on January 20, 2017, I was issued a Business Permit by the City
of Pagadian with business number 4899 and with business address
at Barangay Jamisola Street. Sta. Lucia District Pagadian City

2. However, in the business permit issued in my favor, my surname

therein had been erroneously misspelled as LOGARTO,

3. That my true surname is LAGARTO;

4. That I am executing this Affidavit of Discrepancy to attest to the

truth of the foregoing facts and to explain the discrepancy in my
Business Permit with regards my surname.

IN WITNESS WHEREOF, I have hereunto set my hand this 30th day
