GUARDIAN CONSENT

hereby grant our son/daughter/spouse the permission to undergo training in LIFEBANK
FOUNDATION INC. as one of the requirements in his/her application as Account Officer. We
understand and agree that this is a pre-requisite requirement for Account Officer position of the
company.

We further affirm that we shall not hold LIFEBANK FOUNDATION INC. responsible nor for any
untoward incident, harm, injury that may be caused on our son/daughter/spouse during the
training period of ten (10) days and that he/she will undergo the said actual training without
compensation.
