hereby solemnly declare that the information provided on this form is true and correct to the best of my knowledge. I have read and understood all
the instructions provided in the current prospectus and agree to abide by them. I further undertake that I have sufficient financial resources required
for the program of study at University of Management and Technology.

I also solemnly affirm, declare and undertake that:

I understand that the University may vary or reverse any decision made on the basis of incorrect or incomplete information which I have
provided.
I will submit the Equivalence Certificate from the Higher Education Commission (HEC) or IBCC, whatever the case may be within one week
after commencement of classes.
In case Pakistan Engineering Council (PEC) declines to register me as a Professional Engineer, UMT will not be held responsible for this
action.
I hereby undertake to comply with all requirements of relevant accrediting and regulatory bodies and governmental policies in practice now or
later till the completion of my degree.
I am an HEC Scholar; in case of any fee difference I am bound to pay the remaining fee to UMT.
I shall abide by the HEC rules in case of tuition fee refund.
I will maintain my GPA/CGPA as per UMT policies if the scholarship/financial assistance are being provided to me, otherwise it will be
withdrawn.
I am aware that, if I fail to pass my intermediate / graduation any prerequisite examination with minimum required marks as per eligibility
criteria of the respective program. My admission at UMT will be cancelled forthwith.
In case of cancellation of my admission, I will not claim fee refund and my semester report/transcript of the credits earned shall be cancelled
and will be non transferable.
I shall protect and preserve the property of UMT at all times and shall make all out efforts to prevent others from causing any damage or
destruction to the Universitys property.
I shall fully abide by the decisions of management, various discipline committees constituted for the purpose of ensuring and enforcing
discipline and good order in the university and of other authorities.
I shall abide by the rules and regulations enforced at UMT at present and those which may be enforced at any time in the future.
I am aware that if I could not maintain my CGPA as per the UMT rules, my admission will be cancelled.
I am aware that if I could not complete the degree requirements within the stipulated time period, my admission will be cancelled.
