Describe in detail what happened to each of the four individuals (3 male, 1 female) from the video LOST
YOUTH (you do not need the correct order nor their names). Describe what happened in the incident, what
the resulting injury was and, besides getting proper training & supervision, name 3 things that could have
been done to prevent each injury: /12 marks

Incident & Injury 1: Prevention:


Broken Back Just leave the forklift and take the blame
Crushed by forklift Dont go against what you are told if
Male was crushed by forklift after turning it on while hazardous
not properly operating it Use forklift properly

Lost three fingers Know how to do something before doing it
Tried to clean dough out of machine Be closer to emergency stop button
Female tried to clean out a machine and got fingers Left the dough there
