Dear Sir,
I regret to inform you that my brother Akashdeep Dsa expired on 01Apr14 intestate.

He was employed with your company since 02Jan06 and was entitled for the benefit of Provident
Fund and Gratuity standing to his credit at the time of his death.

I am unaware weather my said brother had nominated my person to succeed to the said amount of
Provident Fund and Gratuity.

I therefore request you to kindly let me know weather in your record my deceased brother had
nominated the name of any person or not. In case if any nomination is done kindly let me know the
balance standing due and credited to the Provident Fund and Gratuity account in honour to make a
petition before the appropriate testamentary court or competent authority established under the
administrator general act to claim the said Provident Fund and Gratuity amount as a legal heir.

Kindly issue to me a letter to the effect that the deceased had not nominated any person if it is found
in your record if no such nomination is done as the same is required furnished for obtaining letter of
administration.
