FORMAL CHARGE
Please be notified that after conducting an investigation at the
instance of the undersigned on the basis of the report forwarded by the
Provincial Human Resources Management Office (HRMO), through a
Show Cause Memorandum dated November 3, 2016 directing the
Respondent Mr. __________ to explain why no administrative case
should be filed against him, the undersigned Disciplining Authority
found a prima facie case for administrative case against Mr. _________.
Mr. _________ is hereby formally charged for Frequent
Unauthorized Tardiness classified as light offense under Rule 46 (F) (4),
of the Revised Rules in Administrative Cases in the Civil Service
(RRACCS), with the following brief statement of relevant facts:
For the first two quarters of 2016, covering the months of
January to June, the Provincial Human Resources Management
Office (HRMO) had been continuously submitting to the Hon.
Governor Jose Ch. Alvarez lists of Provincial Government
Employees who incurs tardiness, regardless of the number of
minutes, ten (10) times a month for every month.
Based on the consolidated list the following are the list of
Provincial Government Employees who incurs tardiness for more
than ten times a month for at least two months for the period of
January to June.

WHEREFORE, pursuant to Rule 5 Section 20 of the RRACCS, Mr.


___________ is hereby directed to submit within 72 hours from receipt
hereof his Answer, in writing and under oath, to this Formal Charge. He
may likewise indicate in his answer whether or not he elects to have a
formal investigation of the charge. Be it noted further that he may opt to
be assisted by a counsel of his choice.
