SERVICE CONTRACT

Conditions as follow:
1. P________per game with Three- Person Officiating (3PO) & P300 per game for
table officials
2. P________per game for every forfeited game.
3. Once the referee arrives at the basketball venue, and the game schedule is being
cancelled due force majeure (e.g. rain, earthquake) the organizing body must pay
P_______per game.
4. The commissioner of the tournament must protect the officials/referees at all
times during and after the game; if the officials/referees are hurt by the player or
team follower, the commissioner is one liable regarding this matter.
5. Commissioner/Game Scheduling Master must inform the officials two days
before the game schedule if theres cancellation of games.

As part of the basketball tournament management the Referee, must follow the rules
of the tournament.

1. If the referee is late five (5) minutes before the game time, the referee will fine
the amount of P50 to the Commissioner as penalty.
2. If the referee is failed to report on the game scheduled, the referee will fine the
amount of P500 to the Commissioner.

SIGNATURES;

The individuals executing this contract represent and warrant that they have the
legal capacity and authority to do so on behalf of their respective legal entities.
