I, Filipino, of legal age, and presently residing at No.55-E Makabayan

Street, Barangay Obrero, Quezon City, after having been sworn to in
accordance with law, do hereby depose and state that:

1. That I am a student of EARIST with student no.

2. That a SCHOOL ID was issued to me by EARIST;

3. Unfortunately, I discovered that said SCHOOL ID was, by

inadvertence, lost/missing.

3. In spite of all efforts to recover the said SCHOOL ID , the same was
nowhere to be found and should therefore be considered lost.

4. I am executing this affidavit to attest to the truth of the foregoing and

for all legal intents and purposes.
