    APPLICATION FOR LEAVE WITHOUT ALLOWANCE ON 14 AUGUST 2017.

    With all due respect, I refer to the above matter.

    2. I first apologize for my behavior that violates discipline as a practical student in this
    section. I am also desperate not to attend the above duty because of family matters that I
    cannot avoid at Kelantan which caused me to take an emergency leave. Besides, I am the oldest
    brother in my family which I shall help my parent with that. And, I plan to return to work the
    day after tomorrow on DATE.

    4 Therefore, with great humility is my hope that you may give me due consideration for
    my application. Your early cooperation and attention in this matter are very much appreciated.
