Instructions

In the Name of the Appraisee column, please fill up the name of the person you are filling the form for.
In the Appraiser column please fill in your name.
In the Filled as column, please mention whether you are filling as a Senior, Junior, Peer or Self

While filling the form the person should be evaluated keeping 12 months into consideration. Very recent happenings or
observations should not be the basis of  judging/rating an individual.
You could get forms to fill  for Self, Peer, Junior and as a Senior. The same would also be mentioned in the mail.

Incase you have not been sent a Self form, it is because people who have not completed at least 3 months in the system as o
31.03.2010, or you are still in your probation period and will not be eligible for an appraisal. This is primarily because assessin
person who is as short as less than 3 months in the system will not be appropriate enough hence people who have joined afte
the 31st December will not be eligible for an appraisal.However their next appraisal will be a prorated one for them.
People who have not completed full 12 months appraisal cycle but have joined before the 1st of Januarywill get an appraisal b
would be prorated keeping in the no of days/months the person has been in the company till the Feb end.
The appraisal is wef 1.4.09.
The HR will try its level best to hand out the appraisal letters by the 4th week of April 2010.

Please ensure that no parameter is left blank
Please ensure that you do not make any change in the form, add or delete parameters
