AFFIDAVIT OF INVESTIGATING OFFICER

I, PO1 FRANCIS A. DACUT, investigator of Agora Police Station, on

my oath as the Investigator-on-Case (IOC), hereby depose and states that:

1. I have personally conducted the examination of the complainant, Ms. Jessy

Mendiola, at Agora Precinct Station, Agora Lapasan, Cagayan de Oro City;

2. Said examination stemmed from the complaint of Ms. Jessy Mendiola

regarding an incident against Armando Mata;

3. Upon hearing the complaint, I personally examined and interview the

complainant and made her to execute a Judicial Affidavit, herein attached as
Annex__;

4. I have faithfully recorded and translated into English language the questions
asked of him/her and the corresponding answers that he/she gave in
response to the questions asked;

5. Neither I nor any other person/s coached this complainant regarding the
answers given by her.

6. That I executed this affidavit to attest the truthfulness of the foregoing facts
and to support the filing of criminal case against Armando Mata for the
crime of Rape.
