GENERAL INFORMATION ABOUT WELLNESS CREDITS

WHAT ARE WELLNESS CREDITS?

Wellness credits provide value to our customers by assisting in longterm management of health care
costs through the implementation of wellbeing programs. They are a means of driving engagement in
wellbeing and care management solutions.

EXPIRATION DATE

Wellness credits have an expiration date and must be used within a benefit year. Unused wellness
credits will be forfeited and cannot be carried over into another plan year, unless stipulated as a
condition of a multiyear contract renewal or sale.

ELIGIBLE SERVICES

Wellness credits are intended to be utilized on services that promote or engage positive behavior
change or sustain health and wellbeing. Eligible services may include educational and fitness programs,
onsite wellness coaching, biometric screenings, wearable monitoring devices, and wellbeing challenges.

USING YOUR WELLNESS CREDITS

Wellness credits may be applied against Independence wellbeing and care management solutions. You
may choose to utilize your wellness credits on any of the well-being services listed and provided by the
vendors in this guide. Independence is already connected with these vendors and can facilitate payment
so you do not have to pay vendors directly for services when utilizing available wellness credits.

WELLNESS CREDIT BALANCES

While the Independence wellness team will assist with verifying the amount of wellness credits
available, it is important for you to keep an accurate record of wellness credit balances. If at any time,
you need to reconcile your wellness credit balance, please contact your Independence Account
Executive.

INCENTIVES

Wellness credits cannot be used towards gift cards or other incentive items associated with a rewards
program; however, wellness credits can be used for any administrative or implementation fees
associated with purchasing the Independence rewards program or other eligible Independence well
being or care management solutions.

VENDOR SELECTION CHECKLIST
Selecting a vendor to fit your needs can be a challenge if you have never done it before. Vendors
should be able to guide you effectively in this process. Based on our years of working with many
vendors, we have compiled a checklist of questions to ask vendors when deciding which vendor is
best suited to meet your needs.

What services do you offer?

What should the participant expect? Are there clothing restrictions?

How much time is required to plan a successful event with adequate participation?

Do you provide an online appointment scheduler? If not, how are walkins handled?

Will you expect a final headcount or a guaranteed minimum number of participants? Are
there penalties for adjusting the final headcount?

What are the qualifications of the personnel and/or technicians that will be staffing the event?

Will aggregate reporting be provided afterwards?

What are the spacing and/or logistical requirements? Are privacy screens included with the cost?

What is the pricing structure? Are their minimum requirements and/or additional costs? What is
the cancellation policy?

Do you require a signed agreement?

REQUEST AN ESTIMATE
We recommend that you contact vendors to request an estimate on services. Pricing structure differs
from vendor to vendor and other miscellaneous costs may apply depending on the details of your
event and the vendor you select. Additional fees may include:

PROTOCOL FOR APPLYING WELLNESS CREDITS

You may work directly with any of the vendors listed in this guide to secure approved and
wellnesscrediteligible wellbeing services. Independence already has established payment
arrangements in place with these vendors so customers do not have to pay vendors directly for their
services.

Step 1 Contact your vendor of choice from this guide and request an estimate of services. Confirm
you have sufficient wellness credits available to cover the services and inform the vendor that
you will be paying for services using your available Independence wellness credits.

Step 2 Conduct your event.

Step 3 After the event, instruct the vendor to send you a copy of the invoice for approval.

Step 4 Once you approve the invoice, instruct the vendor to upload the invoice to Catalyst, the
Independence accounts payable system. All the Independence wellness credit contracted
vendors are familiar with this process. Have them address the invoice to Maureen Conneen.

Step 5 Once the vendor invoice appears in Catalyst, Independence will recognize it, pay it and
deduct the expenses from your wellness credits balance.

If a customer gives an Independence contracted vendor the approval to bill Independence for
amounts that exceed the amount of wellness credits available, the customer will be responsible for
any remaining charges due to the vendor. Any overages will not be paid to the vendor by
Independence.

Cancer Prevention

Our Fight Against Breast Cancer (Online/Onsite; 1 hour)


Vendor: Wellness Concepts

Participants learn about the risk factors that may cause the development of breast cancer and lifestyle
strategies for prevention. The program includes sciencebased nutrition information on specific foods
and nutrients that promote optimal health.

Chair/Hand Massage

Chair and Hand Massage (Onsite; 4 hour minimum)


Vendor: Corporate Touch, Prasada, AREUFIT Health Services, Inc.

The chair and all materials needed are provided. Each massage lasts approximately 10 minutes.
Appointments are recommended every 12 minutes to allow for at least two minutes between sessions
for chair cleaning.

Participants attend a live cooking demonstration and learn to cook seasonal foods healthfully. Classes
are conducted by a registered dietitian and offer relevant nutritional information on the health

Participants learn how to celebrate the holidays without going overboard how to portion and slash
calories from holiday party favorites, dine out healthfully, and balance holiday stress with exercise and
seasonal super foods.

Participants learn how to prepare brown bag meals and snacks in minutes! The program includes
recipes and tips on how to build brown bag breakfasts, snacks, and meals from soup to nuts.

Quick, Easy, Fresh: Cooking with Whole Foods

Participants learn how to use a heart healthy pantry to cook with whole grains, lean proteins, and
healthy oils. The program includes information on recommended foods and products for optimal
health, facts about healthy oils, fiberrich foods, recipes, and sample menus.

Quick, Easy, Fresh: Meals in Minutes (for Holidays or Every Day!)

Participants learn how to prepare quick, easy meals, healthfully! The program includes information on
how to build a heart smartpantry to prepare quick, healthful meals in minutes! Recipes and meal
planning information is included.

Shake It Up: Blend Your Nutrients for Optimal Health

In todays fastpaced, busy world, blending foods is an excellent way of maximizing nutrient intake
when preparing an entire meal is not feasible. Participants learn about the multiple health benefits of
blending foods and ways to incorporate various blends into their diets. Participants receive tastings of
delicious and nutritious shakes along with recipes that even kids will love!

Diabetes

Diabetes and Your Diet (Online/Onsite; 1 hour)


Vendor: Wellness Concepts

Diabetes affects millions of Americans and can be controlled, even reversed, through exercise and
good nutrition. A registered dietitian/certified diabetes educator will provide participants with easily
understood nutrition information and menu planning techniques to improve glucose control and
overall wellness. The program includes information on the physiology of the disease, the importance
of meal timing for glucose control, meal planning tips, and information on the glycemic index.

Energy Management

The Energy Game Workshops: How to Find New Energy and Stop Your Energy Drains (Online/Onsite;
1 hour)
Vendor: Prasada
Participants learn simple breathing, meditation, and stretches for renewed energy.

For online presentations, the client will need internet access to log on to a website, FuzeBox, with a
code provided by Prasada.

Fitness Made Easy for Health Fairs (Onsite; Time TBD)


Vendor: Health Fitness Connections

An exercise demonstration designed to provide individuals with a reasonable alternative to


incorporating fitness with methods of simplicity. It is a fully interactive demonstration that provides
instructions of how to use resistance tubes and your own body weight to address a full body fitness
and conditioning program. Abdominal, lower back, and stretching exercises are also demonstrated
and performed. The program is appropriate for all ages, does not require workout apparel, and can be
conducted practically anywhere.

Heart Health

Cholesterol Counts: Reducing Cholesterol Counts Through Good Nutrition (Online/Onsite; 1 hour)
Vendor: Wellness Concepts, Inc.

Prevent heart disease through diet and exercise. A registered dietitian will educate participants on
healthy eating and positive lifestyle factors to reduce cholesterol. The program includes information
on heart healthy cooking techniques, facts about fats and exercise tips for busy lifestyles. Cholesterol
numbers and triglycerides are explained and discussed. This program provides an excellent followup
to cholesterol screenings.

DASHing to Blood Pressure Reduction (Online/Onsite; 1 hour)

A registered dietitian will discuss the DASH eating plan and provide information on low sodium foods
to help manage high blood pressure.

Heart Health (Onsite; 1 hour)

Coronary Heart Disease (CHD) is the leading cause of death for men and women in the United States.
This seminar focuses on preventing and treating CHD. Participants will learn: the types of CHD, their
risk factors, and how to develop a plan to address their risks and prevent CHD.

Heart Smart: Solutions for Heart Health (Online/Onsite; 1 hour)


Vendor: Wellness Concepts

Participants learn how to improve heart health through good nutrition. Find out the best oils to use
for optimal health, clear up confusion surrounding farmed fish, and learn about an array of
lowcholesterol foods and food products.

The Truth About Carbohydrates and Blood Sugar Basics (Online/Onsite; 1 hour)
Vendor: Wellness Concepts

Confused about carbohydrates? Participants learn how to decipher the many sources of carbohydrates
and how they impact blood sugar levels and energy. Topics for discussion include information on how
to spot a whole grain, facts about the glycemic index and meal planning, meal planning tips, and
snacks for diabetics.

Nutrition Counseling

Independence offers covered employees the opportunity to take advantage of onsite nutrition
counseling at no additional cost.* A registered dietitian can provide your employees with the support
they need to maintain a healthy weight and reduce their risk for developing costly chronic conditions
such as diabetes and heart disease.

Because Independence members are eligible for six free nutrition counseling visits per benefit year
with a registered dietitian, onsite nutrition counseling is a convenient way for them to use this
preventive health benefit.* Select Independence providers will come to the workplace to meet with
employees on an individual basis during the workday to:

Assess their eating habits and personal health risks


Develop a customized strategy to achieve weight loss and fitness goals
Choose appropriate programs and resources to maintain lifestyle changes

How to schedule onsite nutrition counseling
Bringing a network registered dietitian to your workplace is free and easy:
1. Contact your Independence account executive or visit ibx.com to search for a list of network
registered dietitians in your area.
2. You can choose the provider. When contacting the provider, ask if they will come to the
workplace. Schedule the service directly to fit the companys needs.
3. There is no cost to your covered employees for the service; the provider bills Independence
directly.
* Not all employers offer nutrition counseling visits as part of their benefit plans. Please contact Customer Service or your
benefits administrator to determine if this benefit applies to your coverage. If nutrition counseling services are provided in
addition to other covered services, an office visit copay may apply. Deductible and coinsurance may apply to services
obtained out of network.

The Dermascan is an interactive tool that is used to teach participants about sun safety. This screening
allows both the participant and technician to view sun damage on the participants face. Information
on skin cancer and sun safety is provided. Each technician can see 16 to 18 individuals per hour.

Vision

The vision screening is used to assess whether the client needs a more indepth eye examination by
an eye doctor, or if their vision does not need correction. Using the Titmus 2 Vision Screener, staff will
assess visual acuity (near and far), color perception, and muscle balance. Each technician can see
approximately 6 to 8 individuals per hour. This vision screening can be performed alone, or paired
with optometry consultations. This screening is best used in combination with one or more of our
vision seminars.

Fatal Vision/Intoxiclock
Vendor: AREUFIT Health Services, Inc.

The Fatal Vision/ Intoxiclock program was developed to educate people on the dangers of drinking
and driving. The Fatal Vision program gives the participant goggles that simulate elevated blood
alcohol levels. The Intoxiclock demonstrates how a person's level of intoxication changes with
increasing levels of alcohol ingestion, and the rate at which the body metabolizes it. This program
helps educate individuals about impairment and Blood Alcohol Content (B.A.C.). Each technician can
see approximately 10 participants per hour.
