INSTRUCTIONS FOR NON-DEGREE STUDENTS
SECOND SEMESTER, 2015-2016

DATE: January 15, 2016


TIME: 9:00 a.m.-11:00 a.m.

A. FOR NEW APPLICANTS

1. SUBMIT the following to the OCS-CAS, Window 2 to get an admission slip


and Enlistment Survey Form (ESF):

1) letter stating subjects you intend to enroll in (2 copies)


2) proof of graduation
3) official transcript of records or True Copy of Grades (TCG)
4) birth certificate (original & 2 photocopies)
5) marriage contract (if applicable)
6) chest x-ray result
7) 2 X 2 pictures (2 copies)

2. PRESENT the following forms 15 minutes before your scheduled time


at Cybernook, 2nd Flr., Rizal Hall for enlistment, tuition fee calculation, and
issuance of Form 5

1) Admission Slip
2) Enlistment Survey Form (ESF)

3. PAY APPLICATION FEE of PhP100.00 at the Cashiers Office, Old NEDA


Bldg., Padre Faura St,, Manila

4. PAY SCHOOL FEES at the Cashiers office, Old NEDA Bldg.,


Padre Faura St., Manila or ONCOLL option through any Landbank branch

B. FOR OLD APPLICANTS

1. SUBMIT letter of intent to enroll (2 copies)


2. FOLLOW Instructions Nos. 2-4 for new applicants except No. 3

INSTRUCTIONS FOR CROSS-REGISTRANTS FROM OTHER UP UNITS
SECOND SEMESTER, 2015-2016

DATE: January 15, 2016


TIME: 9:00 a.m.-11:00 a.m.

A. FULL CROSS-REGISTRANTS

1. SUBMIT the following to get Enlistment Survey Form (ESF) and FORM 5A.1:

1) UP I.D.
2) Cross-Registration Permit (CRP) duly signed by the College
Secretary and University Registrar of your Mother College
& UP Manila
3) Chest x-ray result

2. SUBMIT the following forms 15 minutes before your scheduled time


at Cybernook, 2nd Flr., Rizal Hall, for enlistment, tuition fee calculation,
and issuance of Form 5

1) Enlistment Survey Form (ESF)


2) FORM 5A.1

3. PAY SCHOOL FEES at the Cashiers office, Old NEDA Bldg.


Padre Faura St., Manila or ONCOLL option through any Landbank branch

4. SUBMIT the following at the Office of the University Registrar (OUR),


UP Manila:

1) FORM 5
2) Cross-Registration Permit (CRP)

5. SUBMIT photocopy of CRP at OCS-CAS, Window 3

B. PARTIAL CROSS-REGISTRANTS

1. SUBMIT FORM 5 (students copy) at OCS-CAS, Window 3 and


FOLLOW instructions Nos. 1-5 for Full Cross-Registrants

GUIDELINES FOR UP MANILA STUDENTS APPLYING FOR CROSS-
REGISTRATION (AFCR) IN OTHER UNITS OF THE UP SYSTEM

1. SECURE & fill-out the Cross-Registration Permit (CRP) from the Office of
the University Registrar (OUR), UP Manila

A) Secure Advisers signature


B) Submit to OCS-CAS the CRP for certification if the subject
is not offered/closed/in conflict with other required
subject/s and for signature of the College Secretary

2. SUBMIT CRP to OUR, UP Manila for signature of the University Registrar

3. SUBMIT CRP to the College where you want to cross-enroll


FOLLOW the guidelines of the said College

4. SUBMIT photocopy of your FORM 5 and receipt of payment to the


OCS of your Mother College

NOTE: A. PARTIAL CROSS-REGISTRATION

Submit the following to OCS:

1) Enlistment Survey Form (ESF)


2) FORM 5 & receipt of payment

B. FULL CROSS-REGISTRATION

Submit the following to OCS:

1) Enlistment Survey Form (ESF)


2) Request letter from the parents
3) Medical certificate validated by the UP Health Service
