SWORN AFFIDAVIT OF ARREST

1.Insp Regi, in legal age and member of the Philippine National Police presently assigned as
Quezon City Police Officer, after having been duty sworn to in accordance with the law, do
hereby depose and state

1. At about 5:00pm in the afternoon, on 10 june2016 Mr. Joseph Ramos appeared in

this office reporting an incident of Republic Act No.8049 or Anti Hazing Law

2. Our Chief Investigator Reginald Pantig directed us to respond proceed at the reported
place of incident to verify the veracity of the said information and affirmative to
conduct an ocular investigation

3. Upon arrival at the place of incident, we were personally informed by the

complainant Leo Carpio that the suspect is Jonas Babierra he is at large

4. We immediately informed our office unit confirming this incident , and thereafter I
together with concerned citizen volunteered to help us to proceed at the house of the
suspect at Bacoor City and that upon arrival there at 10:30 PM in the evening the
suspect was found and we filed the warrant of arrest and the next day the complainant
positively identified the suspect

5. We immediately identified ourselves to Him (suspect) arrested and likewise informed

him the reason of his arrest and his constitutional rights.
 6. He (suspect) submitted himself to us peacefully and further identified himself as one
Jonas Babierra of Bacoor City and that we immediately brought him to our office for
fingerprint examination , and proper disposition

7. We executed on this affidavit statement to arrest the veracity of aforementioned facts

and support the filing charge in court against the arrested suspect
