AFFIDAVIT OF DENIAL

I, JUANITO YUKIT SY, Filipino citizen, of legal age, and resident of Blk. 6 Lot 6,
Sunnycrest Village, Dasmarinas, Cavite, after being duly sworn to in accordance with
law, depose and state: That - -

1. On 17 August 2017, I applied for Court Clearance at Municipal Trial Court of

Dasmarias City, Cavite;

2. Unfortunately, when verified from their records, a certain JUANITO SY

appeared to have been charged of the crime of SWINDLING (ESTAFA)
pending before the Municipal Trial Court of Dasmarias, Cavite under
Criminal Case No. 00-138, Re: 12-28-00, Return: 4-11-06, Arch: 9-506;

3. Through this affidavit I hereby vehemently deny that I am the person

involved in the above-mentioned crime, for I have not committed any crimes,
civil or criminal and I have never been charge in any court, bodies or
tribunals;

4. I am executing this Affidavit to attest to the truth and veracity of the

foregoing and for whatever legal purpose the same may serve;
