Greetings!

Section 15, Article 2 of the 1987 Constitution mandates the protection and promotion of
the right to health of the people, thus, we, as state authorities charged with the custody
of persons, seek to deliver this right among persons under our custody. Pursuant to this,
the undersigned respectfully requests for the issuance of an Order directing us to escort
to hospital of choice for purposes of medical check-up one detainee named Idjao, Virgilio
charged for Violation of Section 5 and 15 of RA 9165, docketed under criminal case
number _____ of before your honorable court. The said person is observed to be
experiencing generalized edema which is pitting at both upper and lower extremities,
difficulty of breathing and scanty urination. Our health service unit suspects a potential
underlying, life threatening condition.

The detainee has no near significant others to contact, prompting us to take appropriate
action by directly requesting this concern to your honorable court.
