Dear Judge Escatron,

Greetings!

As a result of the Mass TB Screening conducted by the joint team of our Health Service
Unit, Department of Health (DOH) and the Philippine Business for Social Progress
(PBSP) in Butuan City Jail last March 2017, more or less ninety (90) detainees were
tested positive for Pulmonary Tuberculosis Disease. Among them, twenty (20) were
advised to undergo a higher form of treatment called CATEGORY II treatment by Dr.
Teresita V. Makiling of the City Health Office. This treatment requires a skin test prior to
the actual initiation of the treatment regimen in order to detect for presence of allergic or
anaphylactic response to the medications. For the said skin test, we are going to need to
transport the concerned detainees to Libertad Health Center for direct observation of the
said test by Dr. Makiling, and escort them back to jail thereafter. The date shall be
adjusted according to Dr. Makilings schedule.

Due to this cause, the undersigned respectfully requests for the temporary transport for
purposes of Skin Testing of certain detainee named APAT, Alberto y Cadenas charged
for Violation of RA 9165, Section 5 docketed under Criminal Case Number 14926 before
your honorable Court.
