2. The reason is because Hospital Umum Sarawak does not keep the record which I
went to on July 2013 due to accidently drink the thinner. My agent do went to the Hospital
Umum Sarawak to get the doctor report and based on the record unit department in hospital,
they say they do not have any record for that.
3. The chronology of the accident is like this, during my work previously as a motorcycle
mechanic at Kota Sentosa, I accidently mistaken the mineral bottle as water. I accidently
drink the thinner. I went to Timberland for treatment and all treatment done in Timberland
and as my employee does not want to pay for the expensive stay at the private hospital, I
was told to refer to Hospital Umum Sarawak for the stay overnight. I went to the emergency
department in Hospital Umum Sarawak and I was given drip and normal check up. I waited
on bed for one night at the emergency department and the next day the doctor ask me if I
feeling good, I say I m fine and was ask to go back. So I did not warded into Hospital Umum
Sarawak and the hospital did not have any of my medical report on that particular incident as
I was treated as out-patient. At the point, I also not given any discharge letter and I went
home straight away.
