Greetings in the line of service!

I am NUP Cleotilde Custodio Delos Santos, presently holding the plantilla appointment as Administrative
Officer III Supply Officer II under item no. 24-001 and SG 14.

I have been working with the Bureau for twenty two (22) years. I was Supply Officer I from 1994 to 2002
and promoted to Supply officer II in the year 2002. I was designated as the Supply Officer of the medical
Dental Service, now HEMD of the BFP National Headquarters for eight (8) years. The height of my
potentials was tested when I was chosen as the Chief of the Supply Office now SAPD of the BFP National
Headquarters on August 2003. My primary task was to handle supplies and properties of BFP nationwide
and established the Property Inventory of the BFP-NHQ . I successfully finished my stint in October 2005.

I was then assigned at BFP Region III as Chief, Regional SAO, and assumed such post until January
2007. As Regional SAO, I was delegated with the function of inspecting, accepting and distribution of all
supplies, materials, equipment, and other properties intended for BFP-R3.

I performed the same functions when I was assigned to BFP-National Capital Region as Chief, Regional
Supply Accountable and Property Division from January 2007 to September 2011. One of the biggest
accomplishment during my stint in the National Capital Region was the successful disposal of thirteen (13)
units of BER Firetrucks NCRwide.

Then on 2012 I was assigned and designated as Chief, PIMS/SAPIS at the Director for Logistics and SAPD
of the National Headquarters and performed the same functions as Supply Officer.

My work experience was supplemented by the various related trainings and seminar I had attended.
Looking through my track record, I had been handling supervisory tasks for almost nine (9) years now as
head of different SAO offices specially as Chief SAO, BFP-NHQ; Chief RSAPD, BFP-R3; Chief, RSAPD
BFP-NCR; Chief, SAPIS/PIMS RSAPD BFP-NHQ.

In reference to Memorandum dtd 01 Sept 2015 I would like to respectfully apply for promotion to
Administrative Officer V- Supply Officer III SG 18 with item No. BFPB-CADOF5-8-2005.
