Blunt injuries of left chest and flank, and right leg. 2. Acute peritonitis. 3. Status post laparotomy incision with sigmoid colon resection, recent. 4, Jaundice. 5. Red discoloration of the left fingers and both feet. 6. Anasarca, 7. Clinical history of spleen laceration, pelvie fractures, and vertebral fractures following motor vehicle collision (anamnestic). 8. Clinical history of coronary artery disease with remote bypass grafting (anamnestic). CAUSE OF DEATH: Complications of blunt traumatic injuries due to motor vehicle collision. MANNER OF DEATH: Accident. OPINION: Jerome Barson, a 78-year-old passenger, was in a motor vehicle collision on 6/9/17. His wife was driving a 2016 Hyundai Accent and crossing an intersection when she collided with a 2010 Toyota Sequoia that cut across the intersection. Mr. Barson was admitted to the hospital with a splenic and mesenteric lacerations and multiple pelvic, thoracic and sacral fractures. He developed multi-organ failure and remained in the intensive care unit until his death on 6/22/17. NubL beer Michael D. His body mass index is 41.0. The body is well preserved and cold. Marked rigor mortis is detected in the extremities. Purple-pink livor mortis is on his back. ‘The decedent is normocephalic without apparent injury to the face or scalp. The decedent has white hair. He is unshaven. The decedent has blue irides with no conjunctival petechiae. Chemosis is present. The scleras are icteric. No facial, nasal, or mandibular fractures are palpated. The nasal septum is intact. No froth or fluid escapes from the nose or mouth. The decedent has no lower teeth and has a few remaining natural upper teeth. There is no trauma of the lips, gums, or frenulum. ‘The neck is symmetrical and has no obvious external injuries. There is no palpable crepitus or hypermobility. No neck masses are observed. ‘The thorax is symmetric. The abdomen is protuberant and has no discoloration or hernias. The decedent is jaundiced. The scrotum is edematous and enlarged. ‘The arms are symmetrical and normally developed. The arms have no needle tracks. The fingernails are intact. ‘The hands are swollen. The left second through fifth fingers are dark red-purple. A 1-3/4 inch red patch is on his right arm. The legs are symmetrical and swollen. The distal ends of the feet and toes are dark red-purple. ‘The back shows a symmetrical external contour, and the spine is straight. The skin shows no rashes. IDENTIFICATIO? No tattoos are on the body. A 6 inch, transverse scar is in his right upper abdomen. A vertical, 8 inch scar is in the midline of his chest. No scars are on his wrists. ‘The decedent is unclothed. ‘An identification band is on his right wrist and bears the name “Jerry Barson”. EVIDENCE OF MEDICAL INTERVENTION: An endotracheal tube enters the mouth. A nasogastric tube enters the nose. A two-port intravenous catheter is in his left supraclavicular skin, Electrocardiogram pads are on the chest and abdomen. A three-port intravenous catheter is in his right arm. A 13 inch, vertical, open, surgical incision is in the midline of the abdomen. Arising from this incision is a drainage tube. The incision is covered by surgical sponges and clear plastic. A fibrinous exudate covers the peritoneal contents. There is an ostomy bag in the left abdomen that contains dark amber liquid. A Foley catheter enters the urethra. 2 NAME: BARSON, JEROME CASE NUMBER: 17-1044 EVIDENCE OF INJURY: A.20x 12 inch, dark red-purple ecchymosis covers most of his left lateral chest, flank, and back. A 6x 9 inch, purple contusion is on the back of his right leg. His right knee and upper leg are contused.
