Activity 12.

4 Conference Evaluation Form 1

The first portion of this form was completed by the student. It has been retyped
verbatim for the purpose of this assessment on 10/14/2017.

Name: Grace S. Date: 10/12/2017

What I learned from this I learned that it is important to consider different kinds of degrees
conference when majoring in music in college, and I should ask questions on
every tour I do.

What I liked about it Miss Meyer made me excited to start applying for colleges!

What I would change Nothing

about the conference

Other comments I would like to explore more about my audition pieces with Miss
Meyer and would like her help in reaching out to accompanists for the
schools that require it.

The second portion has been completed by the teacher.

What I learned from this I noticed that using the letter of recommendation form and scheduling
conference formal conferences makes for a faster turn-around and will allow me
additional time for crafting thoughtful responses on my letters of
recommendation.

What I liked about it The conference allowed me to gain insight as to what the student is
thinking in regards to their future career in music. It allowed me to
focus on her specific needs for a university as a saxophone player.

What I would change In the future, I will hand out forms such as the letter of
about the conference recommendation form and this form earlier, to allow students to
consider their responses to the topics.
