XI. Ongoing Appraisal

Received patient M.A, 33 years old female accompanied by husband on July 20, 09.
Patient delivered a baby girl. Received methergine IM. She was sent to ward for postpartum care.
Monitored her VS q2, temp: 37.1 RR: 21 PR: 98 BP: 110/80. Received medications: methergine
500 mcg, amoxicillin 500 mg and Mefenamic acid 500 mg.
July 21, 09 patient can already walk. Monitored her VS. VS are stable within normal
range, temp: 36.9 RR: 20 PR: 93 BP: 110/80. Received medications: methergine 500 mcg,
amoxicillin 500 mg and Mefenamic acid 500 mg. IE discharge done by POD.

XII. DISCHARGE PLAN

M – Medication

The doctor’s order of amoxicillin 500 mg OD continues for five more days. Medications
should be taken regularly as prescribed, on exact dosage, time, & frequency, making sure
that the purpose of medications is fully disclosed by the health care provider.
E – Exercise

Do passive ROM at home as tolerated by the client. Do ambulation therapy with


assistance and if can tolerate by the client.

T – Treatment

Continue home medications as prescribed. Follow certain instructions provided by the


doctor.

H – Health Education

Regular breastfeeding to help reduce the weight. Health teachings regarding the
importance of proper hygiene especially in the genital area to prevent infection.

O – OPD follow-up

Post partum follow up and family planning as recommended.

D – Diet

Diet which is prescribed should be followed. Soft diet for first few weeks. Eat healthy
foods with vegetables and fruits that will help patient to go back to her normal weight.

S – Signs and Symptoms

Insomnia, dizziness, nausea and vomiting, bleeding, hemoptysis, lethargy, seizures,


dyspnea.
