Advance Cash Request Form Children Provision Charity (CHILD-PRO) 2017

i. This form should be used to request cash in advance of expenditure.

ii. This form is valid only if the requested amount exceeds RM500.
iii. Treasurer will not entertain any request that is out of Treasurers information beforehand.
iv. A confirmation email will be sent to the requestor upon verification.
v. Original receipts should be retained for submission. (Details will be emailed)
vi. Credit/debit card receipts cannot be accepted as proof as they do not itemise goods and
services purchased.
This form must be signed and then returned to the Treasurer.
