Negative Pressure Wound Therapy (NPWT) in the treatment of diabetic


necrotizing fasciitis : a case report.

INTRODUCTION
Necrotizing fasciitis is a rapidly progressive inflammatory infection of the fascia, with
secondary necrosis of the subcutaneous tissues. The speed of spread is directly proportional
to the thickness of the subcutaneous layer. Necrotizing fasciitis moves along the fascial plane.
We present a case of NPWT in the treatment of it.

CASE PRESENTATION
A 58-year old asian female came to the hospital with chief complain of prolonged wound on
her left lower extremity. She has diabetic history. Her physical examination revealed wide
wound, sludge, pus, and necrotizing tissue. We did debridement without NPWT in first
week, her postoperative showed that still the same. And then we did debridement and NPWT
in second week. Her postoperative wound showed granulated tissue less than a week. All
Debridement and NPWT were performed under general anaesthetic.

DISCUSSION
Necrotizing Fasciitis is one of diabetic complications that often end with amputation. It is
often came with productive wound. The idea is how to clean the product? And NPWT does
answer it. NPWT has already applied in a few center, but less literature. The present case
report demonstrates the application of NPWT particularly in necrotizing fasciitis.

CONCLUSIONS
NPWT can be applied to get the products of wound cleaned.
