SAFETY ALERT

GRINDING WHEEL FATAL INCIDENT- NON TCM PROJECT RELATED


Brief Description of Incident:

Few days ago, a grinding wheel related incident has


been occurred in another project, NOT TCM related.

The grinder wheel shattered during the use and a large


fragment of the wheel went into the workers neck
which resulted sever injury and death of the worker.

SAFE WORK PRACTICES:


All workers using grinders shall receive Abrasive Wheel Safety Awareness training;
Disconnect power supply before changing the grinding wheel and/or adjusting/replacing any
accessories;
Select the correct grinding wheel based on the operating speed of the grinder, the type of grinder being
used, the type of work being performed and the manufacturers recommended practices.
Ensure wheel speed (RPM) is higher than grinder speed (RPM). Never use low rated RPM wheel on
high rated RPM grinder;
Be sure the wheel fits freely on the spindle;

Do not remove the guard from the grinder; Never use a grinder without guard fitted on it;
Do not remove the handle from the grinder; Never use a grinder without the handle fitted;
Always check the expiry date of the grinding wheels and perform a Ring Test before installing the
grinding wheel;
Allow grinder to come to full speed before contacting work piece;
Maintain steady pressure on the work piece. Sudden bumping, pounding or impact could damage the
wheel
Do not apply excessive pressure to the wheel. So that the motor slows noticeably of the work gets hot,
could stress the wheel, overheat the work piece and reduce your control.
Never attempt to grind in an awkward position. A portable grinder can kick and glance off the work if not
properly controlled.
Never hammer on a grinding wheel to tighten. Hammering on the wheel could damage/crack the wheel.
Tighten only with the tool supplied with the grinder.
Always use appropriate personnel protective equipment.
Allow grinder to come to complete stop before removing PPE or setting tool down.
Only use grinders with a cut-off or dead-man switch (i.e. switch that cuts off power once finger
released), NOT lockable or conventional dead-man switch.

Youd never think a grinder could kill you. But it can!
