Then we realized that TOF could be successfully repaired at any age—in the newborn period
(the first 30 days), or in infancy (the first year of life). This in turn led to what came to be
know as the Castan˜ eda doctrine.40
Its main tenets were:
1. Repair TOF whenever the patient needs it, at any age—in the newborn period, if
necessary 2. One operation is better than two; whenever possible, avoid palliative surgery
such as Blalock-Taussig shunts. Thus, an accurate understanding of the “monology of
Stensen”7 cast a long and important management shadow.
