FIELD TRIP APPROVAL
Consent is hereby requested for ________________________________________ to go with the
class/group to Amicus Castle Theatre, Budd Lake, NJ on Monday, November 20th, 2017 under
the supervision of the 8th grade team
I understand that this trip will enhance our study of Edgar Allan Poe, his tragic life, passions
and breathtaking tales.
1.) Departure from school 8:30 A.M.
2.) Return to school approximately 1:00 P.M.
Cost of the trip is $22.00 (Checks or money orders only, NO CASH) Please remit with signed
consent form to homeroom teacher in a sealed envelope labeled with students name and
homeroom. Checks must be made payable to Lakeside Middle School.
Other comments: Students need to bring a brown bag lunch.
Approval Return Date: Friday, October 27th, 2017
NO FAXES OR PHONE CALLS WILL BE ACCEPTED.
Please be advised that students who are absent on the day of the field trip will not receive a
refund unless the company offers one.

(Complete and return lower section with a check for $22.00 made payable to Lakeside Middle
School to homeroom teacher by Friday, October 27th, 2017. Keep upper portion for your records.
