AFFIDAVIT
I, EULALIA O. DELA TORRE, of legal age, widow, Filipino citizen and with
address at Lanigay, Polangui, Albay, after having been duly sworn in accordance with
law do hereby depose and state that:

1. I am the duly elected Treasurer of Light of Life Christian School of Polangui

Retirement Plan, Inc., a non-stock corporation which was issued a
Certificate of Incorporation on November 9, 2017.

2. The trustees and officers of the non-stock corporation will not receive any
income, compensation or any emoluments from the corporation.

3. I am executing this affidavit to attest the truth of the foregoing

statements and for the purpose of complying with the requirements of
Bureau of Internal Revenue for tax exemption and for whatever legal
purpose it may serve.
