SITE VISIT QUESTIONNAIRE

Occupational, Safety and Health Guide Questions:

1 Is there first - aid kit in the site?


Is there fire extinguisher?
2 Is there Project Engineer/ Resident Engineer on the site everyday?
3 Is there any Safety Officer on site who keeps the safety at all times?
4 Temporary facilities such as Field Office for engineer included in the item of work? How much money is allotted?
5 How many construction workers does the project has?List and identify each worker according to its work designation.
For example: Skilled Workers and Construction Laborers. Under Skilled wrokers are Leadmen, Carpenter, Steelmsn, Mason, Tile Setter, Welder, Painter, M
Construction Laborers/ Helpers - Warehouseman, Time Keeper, Purchasing Officer
6 Identify the hazardous scenarious or events that you observe. Provide mitigation plan per event.
7 List down the safety practices adapt in the site.

8 What is the project name?


9 Who is the contractor? If governmnet agency, what agency ? If private construction firm, provide the Name, address and contact no.
10 Who is the recepient of the project?(if government). Who is the owner or client's name?(if private)
11 When is the start of work or also known as date of effectivity? What is the duration? When is the last day of work or the expiry date
12 What are the problems encountered before, during and after the construction process?
13 What are the immediate action untertaken to avoid negative slippage and that the project is on time or advanced?
14 Is there any sentiments coming from the owner? List down positive and negative observations. List down the responses of the contractor per item of wor
15 Comfort room for construction workers availlable and accessible?
16 Does potable water supply available?
