N479 Professional Activity Evaluation Form

Describe the community service/professional activity/s you attended or participated with (be
specific about the purpose and your role):

During my eight hours of professional activity/ community service I did two activities. One

activity was held at St. Lukes home and consisted of inter-professional teamwork between

medical students, nursing students, nutrition students, social work students, and pharmacy

students. The students were put into teams with members from each field and were given the task

of interviewing geriatric residents at St. Lukes Home. Students interviewed the residents about

their moods, screening for depression and suicidal ideation. Each student took on different

questions during the interview and intervened when appropriate according to their role.

For the Stop the Bleed course held by ICSAVE, we were lectured on how to stop emergent

bleeding, how to utilize tourniquets, and how to safely remove an unconscious person from an

unsafe situation. There was a lot of hands-on activity where we were able to practice these newly

learned skills. My role in this course was to learn how to act in emergency situations and how to

control bleeding.

Where was this held and what was program/activity length?

The St. Lukes IPEP Healthy Mood was held at St. Lukes home and was 4 hours long.

The Stop The Bleed course was held in the UA College of Medicine and was 4 hours long.

Discuss the value of this experience to you (be specific).

The Stop The Bleed course was of value to me because the skills and knowledge I gained could

be applied to an emergency situation and hopefully save someones life. The St. Lukes IPEP
training was of value to me because I want to work with the geriatric population in the future. I

loved spending time with the residents at St. Lukes and hopefully was able to shed light onto

some issues with anxiety, depression, and suicidal ideation among the residents.

Would you recommend this specific community/professional experience in the future; why or
why not? (Describe)

I would definitely recommend both of the community and professional experiences in the future.

The Stop The Bleed course was very informative and had great instructors. We learned a lot and

became confident in the skills we were taught. These skills can be used in emergency situations

by civilians and medical health professionals alike.

I would also recommend attending St. Lukes IPEP course. This course allowed me to work with

several other members of an inter-disciplinary team, allowing me to see everyones skill set and

priorities. This training also allowed me practice asking questions about mental health among

members of the geriatric population, which I had not done of much before.

A brochure and certificate was obtained for the Stop The Bleed course, as well as a signature on

the brochure. I forgot my clinical hours form for both of the professional activities. Certificate

and brochure attached below.

I did not obtain a brochure for the St. Lukes IPEP but several professors were present and would

be able to attest to the fact that I was present (Susan Markovich, Colleen the TA). I do not have

any sort of signature for the St. Lukes IPEP experience.
